Advisory Committee.

I understand the commitment that my child and I are making, and I will help my child follow this agreement. I (we) have reviewed this application and agreement form including identified responsibilities and expectations and give consent for his/her participation. It is expressly understood and agreed that MOXI, The Wolf Museum of Exploration + Innovation and its staff, volunteers, board members, representatives and agents shall not be responsible or legally liable for any losses of personal property or for any bodily injuries, or the results thereof, incurred and suffered by the Committee Member in connection with his/her participation in the program. By signing this agreement, I (we) hereby waive any claims associated with this program.

PHOTO RELEASE

I grant MOXI The Wolf Museum of Exploration + Innovation the right to take photographs and/or video of me and my family for use on the museum's website, social media channels and publications. I authorize MOXI, its assigns and transferees to copyright, use, and publish the same in print and/or electronically.

I agree that MOXI may use such photographs or footage of me with or without my name and for any lawful purpose, including such purposes as publicity, illustration, advertising, and web content.
